Hacia un nuevo Protocolo para la Planificación Anticipada de Cuidados en casos de prematuridad grave. Una perspectiva personalista

Las decisiones más graves y complejas desde el punto de vista ético se refieren a los bebés extremadamente prematuros (< 27 semanas de gestación), en cuyo caso se puede considerar tanto el soporte vital activo como los cuidados paliativos en el marco de la planificación anticipada de la atención médica (ACP). Esta planificación surge cuando la supervivencia es incierta o la calidad de vida futura suscita serias preocupaciones. En este proceso, los padres, los médicos y los equipos sanitarios deben decidir por un paciente incapaz de expresar su voluntad, mientras que los comités de ética clínica pueden proporcionar orientación. Este artículo examina estos dilemas desde un enfoque bioético personalista, introduciendo el Protocolo Personalista de Planificación Anticipada de la Atención para “bebés extremadamente prematuros”/EPI (PACP-PEPI). Este protocolo dinámico de siete pasos aboga por el diálogo, la proporcionalidad, la prudencia y la revisión continua como fundamentos éticos para una atención neonatal compasiva y coherente que salvaguarde la dignidad y refuerce la confianza de los padres.
